Senior Social Worker, Integrated Care

An exciting opportunity has arisen for 2 Senior Social Workers to join our services, with the emphasis upon managing the links between health and social care. We are looking for motivated, autonomous and creative applicants to drive relationships and networks with health, and to support integration between our services.

Wokingham Integrated Partnership Programme are running a number of innovative programmes to promote integration and develop proactive preventative work. We are therefore looking for applicants to progress and maintain excellent relationships and links with Primary Care Networks, GP Practices, Community Health Services, hospitals and Voluntary Care Services. This opportunity to provide joint working, training and problem resolution will result in greater resilience across the whole system, with a shared knowledge of working practices and access to information and support. Wokingham Borough has established Multi-Disciplinary Meetings across health, social care and the VCS. Part of the role will be to chair these meetings and to contribute to their ongoing development and success. 

The posts will be based within the WBC Adult Social Care long term and short term teams in order to embed this way of working. Alongside this development role, the post holder will also be required to hold a small caseload of cases generated from MDT referrals, to support the wider teams and provide supervision to a maximum of 2 Social Workers.

The post holder will be a qualified Social Worker and registered with Social Work England, with experience of working in adult social care. They will have a good knowledge and experience of working with all relevant legal frameworks. The ability to work in a busy, fast paced environment and to be able to plan and adapt to change is also essential. 

We are a supportive and established service, offering a robust induction programme, regular supervision, and good opportunities for CPD training and development.
