We’re pleased to offer a new opportunity for a Social Care Practitioner to join our West Team in Adult Social Care at Wokingham Borough Council. This is a full‑time, permanent role.

As Social Care Practitioner, you’ll work with adults and their carers to help people live as independently as possible and stay safe in their community. You’ll focus on what matters to each person and what they can do, building on their strengths rather than what they can’t do.

You’ll carry out direct work with people, with support and guidance from a registered practitioner. You won’t be doing this alone; you’ll be part of a supportive team that works together to make a positive difference.

Adult Social Care supports adults who need extra help to stay healthy, feel connected to their community and live a fulfilling life.

This role involves working directly with adults with a wide range of needs, including older adults, people with physical disabilities, individuals affected by alcohol or substance misuse or hoarding, as well as carers who are seeking advice, support and guidance. 

This role also offers a clear development pathway, with the opportunity to progress to a Grade 6 once all progression criteria are met.

Key responsibilities:

  • Managing your own diary and caseload, making sure work is completed within agreed timescales and according to practice standards
  • Carrying out initial care and support needs assessments and reassessments in line with our legal duties of the Care Act 2014
  • Support planning with a strengths-based and risk management approach
  • Identifying informal carers and undertaking carers assessments in line with our legal duties of the Care Act 2014
  • Identifying safeguarding concerns and reporting these accordingly
  • Gathering and bringing together feedback from different people involved, including working with advocates when needed
  • Writing clear reports and keeping case records up to date
  • Communicating well with the people we support, listening carefully and treating everyone with respect
  • Working closely with colleagues and contributing as part of a team

Candidate requirements:

  • Confident IT skills, with the ability to learn and use adult social care recording systems
  • Clear and effective verbal and written communication skills, including communicating with people who have different or complex communication needs
  • The ability to build positive relationships with people from a wide range of backgrounds
  • An understanding of how ill health, disability, social disadvantage and poverty can affect a person’s wellbeing
  • Experience of working in a health or social care setting, or in a customer‑facing role
